Since our first annual conference in San Francisco, in 1999, the Care Continuum Alliance has been a leader in high-quality educational programming and exhibit events for the population health improvement community. Today, population health improvement stakeholders attend our annual Forum to learn about innovative program designs, operation, outcomes measurement, methodology and other timely topics. The Forum 11, our 13th annual meeting, blends quality educational sessions, our Leadership and Quality Impact awards programs, networking events and the largest exhibit hall in population health management into a premier event that draws senior-level decision-makers from a broad variety of purchasers and payers, including health plans, employers and government. Care Continuum Alliance members receive discounts on conference registration and workshops. Member organizations also receive special pricing for exhibit booths and prominent recognition in conference materials. Our educational programming extends beyond our annual conference. To keep pace with the population health community's demand for new educational content, the Care Continuum Alliance periodically presents audio conferences and Webcasts to highlight emerging issues and educate new audiences, such as employers. We also partner with other leading organizations, including the Patient-Centered Primary Care Collaborative, Partnership to Fight Chronic Disease, American Association of Preferred Provider Organizations and the Society of Actuaries, on shared resources and joint event sponsorships. |
