The Care Continuum Alliance thanks the dedicated individuals who will share their expertise with the population health improvement community through presentations at The Forum 11. Every professional who contributes to educational programming helps advance understanding of chronic disease care and improve outcomes for patients and other stakeholders.

WHAT YOU CAN DO FOR US
To ensure worry-free preparation of presentations, the Care Continuum Alliance asks its faculty to:
- Submit slide presentation and handouts to CCA by Thursday, August 11, 2011. The conference is a paperless meeting and presentations will be shared with attendees online prior to the conference, and attendees will receive a USB drive with .pdf versions of all slide presentations upon checking in at the registration desk. Do not make changes to your presentation after Aug. 11, as attendees look forward to receiving your final presentation.
To minimize commercialism, faculty members must use the official Forum PowerPoint template for presentations. Company logos are only permitted on the first and last slides of PowerPoint presentations. Failure to use the official Forum PowerPoint template for every slide of a presentation will jeopardize future acceptance of proposals from your organization.- The Forum 11 Presentation Guidelines

- Review your presentation description online by Friday, July 1. The Care Continuum Alliance reserves the right to edit all presentation titles and descriptions. Please check the conference Web site to review your session--this description will appear in the onsite print program. - Update your contact information. If you've recently changed jobs, locations or companies, please forward updated information The Care Continuum Alliance. As a faculty member, you will receive a complimentary registration based on the contact information submitted with your abstract proposal.
- Make your hotel reservations immediately. The Care Continuum Alliance has secured a limited number of conference-rate rooms at the Hilton San Francisco Union Square. To reserve a room, visit the Hotel page of this site and use the log-in and password you received through in your registration confirmation email. NO COMMERCIALS, PLEASE
Presentations must be free of commercial content or endorsement. Should a presentation be identified as being commercial, the presenter will not be invited back.
The Accreditation Council for Continuing Medical Education (ACCME) expects that the content or format of CME/CNE/CPE activities and related materials will promote improvements in quality of health care and not a specific proprietary business interest of a commercial interest.
Therefore, during your presentation, please adhere to the following three important points:
- CME/CNE/CPE must give a balanced view of therapeutic options. Use of generic names will contribute to this impartiality. Avoid the use of trade names if at all possible. If your CME/CNE/CPE educational material or content includes trade names, trade names from several companies should be used where available, not just trade names from a single company.
- To ensure the best interests of the public, it is important to utilize and reference the best available evidence, when preparing your presentation.
- No advertising is allowed on any slides or materials. Remove company names from slides and materials; for example, if they run along the bottom of a slide. Only direct references to the data being shown may run along the bottom of the slides.
Your presentation should also be compliant with the ACCME/WNA(ANCC)/ACPE content validation statements, which are:
- All the recommendations involving clinical medicine in a CME/CNE/CPE activity [are] based on evidence that is accepted within the profession of medicine as adequate justification for their indications and contraindications in the care of patients.
- All scientific research referred to, reported or used in CME/CNE/CPE in support or justification of a patient care recommendation [conforms] to the generally accepted standards of experimental design, data collection and analysis.
