Who Can Present

While anyone can attend The Forum 12, only Care Continuum Alliance members can present during concurrent sessions. The Forum 12 will provide presentation opportunities for non-members, including through a poster competition and sponsored symposia to supplement the peer-reviewed, juried process. Presentation Tracks

CCA is accepting presentation proposals in five Forum 12 concurrent session tracks:

  • In the Literature: recognizing outstanding peer-reviewed journal articles on population health management strategies published during the 2011 calendar year. These articles can be from U.S. or international journals, but cannot be commentary or opinion pieces.
  • Measuring Value: highlighting advances in the measurement and outcomes impact of wellness, chronic care management and broader population health management programs.
  • Collaborative Strategies: illustrating new collaborations in health care delivery and coordination among all types of health care providers (both physician and non-physician) and payers. These strategies and collaborations will represent advances in implementing new care delivery models that focus on coordination, patient engagement, new payment models and strategies, and outcomes.
  • Innovations in Care: showcasing the best new innovations and technologies for improving care, engagement, coordination and outcomes for all populations, including high-risk and unique populations, such as Medicaid and oncology.
  • Advancing Wellness: demonstrating value and best practices to promote engagement in healthful lifestyles in the workplace and other settings.

As a provider of continuing medical education (CME), the Care Continuum Alliance strictly follows the Accreditation Council for Continuing Medical Education guidelines and expects that the content will promote improvements or quality in health care and not a specific proprietary business or commercial interest.

  • Sponsors are not involved in the planning, content or execution of educational programming.
  • Presenters must adhere to the following important points for their presentations:
    • Speakers must give a balanced view of therapeutic options. Use of generic names will contribute to this impartiality. If content includes trade names, trade names from several companies should be used where available, not just trade names from a single company.
    • All the recommendations involving clinical medicine are based on evidence that is accepted within the profession of medicine as adequate justification for their indications and contraindications in the care of patients.
    • All scientific research referred to, reported or used in support or justification of a patient care recommendation conforms to the generally accepted standards of experimental design, data collection and analysis.
